Our online forms have changed

The company that supplied our old website and online forms has gone into administration, so our old forms have closed. Please use our new online request service for medical problems, fit notes, medication and admin requests. You can also use the NHS App.

Confidentiality and medical records

Patients may access their medical records. If you would like a copy of your medical records, the request must be made in writing and the practice has to respond within thirty days. Alternatively, you can view these instantly on the NHS App. If you would like to set this up please use the register for online services form.

No one has the right to be told the medical history of another person, except the parents of very young children. We are unable to confirm whether a patient is registered at the practice or divulge information about adult relatives without their consent. Complete a third party consent form.

We share your medical records with other services treating you. You may choose to decline to have your information shared in this way by completing our shared care form. Please note that this could disrupt your care.
